Meet the Conditions

Make sure your medical conditions meet the Michigan Medical Marijuana Act. This includes diseases such as HIV/AIDS, Cancer, Alzheimer's, Amyotrophic lateral sclerosis (ALS), Crohn's disease, Glaucoma, Hepatitis C, and Nail-patella syndrome. You must be 18 years or over to meet to qualify for a certification card.

Schedule an Appointment

Arrange a time to meet with our physicians and get their opinion on whether you should receive medical marijuana. Should you qualify, you will receive a written certification stating you qualify for the use of medical marijuana.

Fill Out Application Form

You must complete and submit your MMMP Michigan medical marijuana application form along with a form of residency proof (a driver's license for example). You will also pay a fee for your certification card and caregiver application (if possible)
